Is my GFX card on its way out?

The computer stays running, and I just get a black screen, and the monitor light turns orange as if its go no video signal, any sound playing starts to freeze and stutter. I have to hit the PC reset to reboot and the display returns.

I have switched the monitor to the IGP and it seems to have stopped the black screen. I also get a random horizontal line that keeps appearing in different places on screen when thru IGP.

System as in signature, with no overclocks.
 
that might even be the monitor being broken, have you tested different monitors?
is there any specific scenario where these freezes happen?
 
My PSU is OCZ ZX 850.

I switched my monitor from the IGP to the gtx580 and in less than a minute, it black screened, sound stuttering and pc froze.

While its running thru the IGP its not black screening or pc freezing. I just get random flashing horizontal lines every now and then on screen while connected to IGP.

I don't get any flashing horizontal lines from the gtx580.

nVidia drivers are current.
 
Strange that you got horizontal lines on igp, might be problem with ram, but I also think this could be your psu
 
I also think it could be the PSU. Try re-seating everything and see if it still does it. Also, how long has it been doing it for?
 
It started in the past 2 weeks. Everything has be fine till then.

It started to sometimes randomly reboot, in windows and when starting a game. The screen would always go black first. Sometimes USB devices would just keep unplugging and plugging into windows. So I have to end up pulling out the USB cable.

Yup, it looks like to the power supply not being able to supply enough power to USB on 5v rail.
And possibly other rails too (12v)
Run OCCT power supply test and post the graph here

I have been running stable on the i5 IGP and i swapped out the gtx580 and put in my spare AMD 6850. Both have run the OCCT PSU test without any issues.

With the gtx580 in, I just got a black screen and frozen PC before the test started properly.

I had the same problem once. Windows would shut down randomly from time to time.. then after a few months it was after 10mins.. then 5.. then barely after logging in.

I swapped out my psu for a spare i had and used the same cables. Still happened. So i switched the cables around to a entire different set and problem went away. Went back to my other PSU and the problem was still there. I unplugged the sata cables and while i couldn't boot into windows, i could leave the PC in the Bios(i left it there for 4 hours just to make sure). So turned out one of the rails went died. No matter which sata port i plugged in it would not boot. Got a new PSU and it resolved my problems(just make sure to use the cables supplied with the PSU as some are not compatible with others).

Just try what i did and switch out any cables you can. Worked for me to figure if any of the PSU ports/rails were dead.
