Mach 2 GT fan wiring diagram needed

I've just bought a modded mach 2 GT, the fan wireing is not origional and after flashing the controller i get the message fan fail on the lcd and then the mach shut's down so if someone could help me out with a wiring diagram so i can make sure the wire's are going to the correct spots (also the yellow speed line from the fan is only coming from the rear one which has been split into two to simulate it's coming from two which i would also like to change back to origional).

Thanks for that, got it sorted now . Although i think there are other problems, need to play a little. I get a temp error on it unloaded.
