CPU-Z & CC cleaner reporting wrong GPU?

Hey just replaced my old 4870 with a 6950 however, most programs inclusing, 3D mark 11, CPU-Z & CC cleaner report the card as a 4870?. I also keep getting display driver stopped responding but recovered errors whilst gaming. This seem like a software error to me so i uninstalled and cleaned drivers with driver sweeper in safe mode and installed the latest drivers 11-12 however the issue still persists any suggestions?

Have found a soloution to the first problem. for anyone else suffering from the same issue the old graphics card must be uninstalled from device manager however, a tweak is needed to see the card copy the following commands into the command line:

set DEVMGR_SHOW_NONPRESENT_DEVICES=1

start devmgmt.msc

Then on device manager select "Show hidden devices" option and uninstall the old graphics card. Then do a full reinstall of the graphics card and the card should now be reported correctly. Will report back after testing to see if it fixes the "display driver stopped responding but recovered error".
