Facebook Security ?

I keep losing access to my Failbook account, I haven't a clue who's doing it as my password is saved nowhere other than my computer at home and at work no passwords are saved on the computer.

Anyway, Is there possibly an app on a smartphone that links to a Facebook account that requires the account to input a code from the linked app before allowing you to login ?
